Does your provider uses 3Com equipment? Do you have net.graph.nonstandard_pppoe
in your /etc/sysctl.conf?

Assure that you have /usr/sbin/ppp, ng_pppoe.ko, netgraph.ko and kernel rebuilt
from new sources, and none of them is stale.

Can you show problematic parts of ppp.log?
