Today I've hit this panic when playing with debugger. The exact sequence
was:

~# -- (break to debugger)
db> break carp_ioctl
db> cont
(several seconds out of debugger)
  [break in carp_ioctl]
db> trace
...
...
db> next
   panic: sched_add: kse 0x???????? (???) already in run queue
   cpuid = 1
   KDB: enter: panic
