USB Attach Failures (USB_ERR_STALLED)

Hi,

As of yesterday (not yesterdays current) I have been having issues when 
booting my D630 on the dock and having the USB devices attach correctly, 
this was all working before the weekend and I have not changed 
anything.  My src tree is at r197709.

If I disconnect the usb keyboard and mouse then re attach them to the 
same ports on the dock they are detected correctly. 

Is this a known issue?
