Arrgh! My mistake - I showed generic
/usr/ports/www/apache22/Makefile.options instead of
/var/db/ports/apache22/options. So here's the correct one:

    # This file is auto-generated by 'make config'.
    # No user-servicable parts inside!
    # Options for apache-2.2.16
    _OPTIONS_READ=apache-2.2.16
    WITH_THREADS=true

And just for case output of of apr1 options file:

    # This file is auto-generated by 'make config'.
    # No user-servicable parts inside!
    # Options for apr-ipv6-devrandom-gdbm-db42-1.4.2.1.3.9_1

    WITHOUT_LDAP=true
    WITHOUT_MYSQL=true
    WITHOUT_NDBM=true
    WITHOUT_PGSQL=true
    WITHOUT_SQLITE=true
    WITH_DEVRANDOM=true

As you see, threads are enabled in both, but still it shouldn't have any
effect on mod_cache, but only on mod_mem_cache, which is _disabled_.
