  I have same problem with Panasonic KXL-RW32AN USB CD-RW Drive.
  with uhci and ohci.
  (it works on 4-stable...)
umass0: GENERIC CDRRW09 USB CD-R/RW drive, rev 2.00/11.10, addr 2
umass0: Phase Error, residue = 95

  I use it on 5-current with below durty patch. Try it.
